I am a caseworker/law guardian. How do I refer one of my clients for your services?
We receive referrals as early as 1 month prior the young person's exit from foster care (i.e. turning 21, return to parent). Before we can accept the young person, you must fill out the Referral Form and return it to us. We would prefer to receive the completed referral form earlier rather than later. This allows us enough time to meet with the referral and assist with planning his or her transition out of child welfare.
